Requirements for Enrollment

For NEW/TRANFEREES:  

​

1. Report Card or Form 138 (original copy) with eligibility to transfer signed by the principal
2. Photocopy of PSA or NSO Birth Certificate
3. Certificate of Good Moral Conduct from either Principal, Guidance Counselor/Adviser.

For Old Students:

Procedures for Enrollment
Parents may enroll their children thru ONLINE or in School Enrollment. For ONLINE Enrollment see the procedures below.

View your grades for S.Y. 2019-2020 students. Click the link below to view your grades. For NEW and TRANSFEREE, you can still enroll via online.

Fill up online ENROLLMENT FORM

Bring your requirements for validation.
