Requirements for Enrollment

For NEW/TRANFEREES:

1. Report Card or Form 138 (original copy) with eligibility to transfer signed by the principal
2. Photocopy of PSA or NSO Birth Certificate
3. Certificate of Good Moral Conduct from either Principal, Guidance Counselor/Adviser.

For Old Students:
